Motoko is the main protagonist of the novel Otaku, which was first published in Japan in 1992. Motoko is a very attractive woman and has a rebelliousness about her; her attitude towards cyborgs makes her very different from most of the other heroines of the original manga and anime.

In Otaku, Motoko is a terrorist. She hacks into the government's central computer and paralyzes it. She then proceeds to destroy the entire system in order to deprive the public of essential services that the government uses to control them. Since the GITS movie (and Stand Alone Complex for that matter), Kusanagi has taken on a more mysterious role and was briefly seen on the front cover of an album in Ghost in the Shell. She has been seen at the end of the movie, but not in full. At the end of the movie, the Major was revealed to be the leader of the Public Security Section 9 (PSS 9), and her bandage has been peeled off.
